What was done

Retrospective descriptive epidemiological study identifying all in-game Achilles tendon ruptures (ATRs) across 7 NFL seasons (2018–2024) using league-wide electronic medical records. Researchers evaluated video footage alongside time-aligned player tracking metrics (speed and motion direction) and player/game data to classify contact mechanisms, body positioning at injury, situational movement scenarios, and injury distribution across player characteristics.

What was found

Across 7 seasons, 77 in-game ATRs occurred. Ruptures were predominantly noncontact (64%) or indirect contact (35%) and occurred at relatively low translational speeds. Three primary movement scenarios accounted for 97% of cases: change of direction (40%), overload (30%), and rock back (27%). Most injuries shared a characteristic body position: the injured leg extended behind the body, ankle dorsiflexed, and hip and knee extended. Higher injury rates were noted in defensive players and athletes with >3 years of NFL experience (exact rate ratios and confidence intervals not reported in the abstract).

Why it matters

It demonstrates that elite football Achilles ruptures stem primarily from high propulsive push-off loads during low-speed noncontact postures rather than direct collisions, pointing toward specific movement strategies (like rock-back motions) as modifiable prevention targets.
