What was done

A systematic review and random-effects meta-analysis (PROSPERO CRD42022324070) of databases including MEDLINE/PubMed, Cochrane/CINAHL/EMBASE, and Web of Knowledge up to May 1, 2022. The authors included three-arm randomized crossover trials comparing the acute effects of pre-meal exercise, post-meal exercise, and a no-exercise control on postprandial glucose excursions in individuals with and without type 2 diabetes. Risk of bias was evaluated using the Cochrane RoB 2 crossover extension, and effect sizes were calculated as standardized mean differences (SMD, Hedges' g).

What was found

Eight crossover RCTs (30 interventions, 116 participants: 47 with type 2 diabetes, 69 without) were included: - Post-meal exercise significantly reduced postprandial glucose excursions compared to pre-meal exercise (15 effect sizes; SMD = 0.47, 95% CI [0.23, 0.70]) and inactive control (15 effect sizes; SMD = 0.55, 95% CI [0.34, 0.75]). - Pre-meal exercise showed no significant reduction in postprandial glucose compared to inactive control (15 effect sizes; SMD = -0.13, 95% CI [-0.42, 0.17]). - Shorter latency between meal ingestion and exercise significantly moderated greater reductions in postprandial glucose (estimate = -0.0151, SE = 0.00473, Z = -3.19, p = 0.001, 95% CI [-0.024, -0.006]).

Why it matters

This review clarifies exercise timing for glycemic management, showing that physical activity performed soon after a meal blunts postprandial glucose spikes, whereas exercise before a meal does not significantly alter the immediate postprandial response.

Limits

The total sample size is small (116 participants across 8 studies). All included trials were judged to have a high risk of bias. The analysis only examined acute glycemic excursions from single exercise sessions, without data on long-term clinical outcomes (such as HbA1c) or sustained metabolic adaptations.
