Association of wearable device-measured vigorous intermittent lifestyle physical activity with mortality.

What was done
Researchers analyzed accelerometer data from 25,241 self-reported nonexercisers (mean age 61.8 years, 14,178 women and 11,063 men) in the UK Biobank to assess the relationship between wearable-measured vigorous intermittent lifestyle physical activity (VILPA)—short bursts of daily vigorous movement lasting 1 to 2 minutes—and all-cause, cardiovascular disease (CVD), and cancer mortality over an average 6.9-year follow-up. Findings were compared to vigorous physical activity (VPA) in 62,344 regular exercisers (1,552 deaths).
What was found
VILPA showed a near-linear inverse association with all three mortality outcomes across 852 recorded deaths in nonexercisers. Compared to zero VILPA, a median frequency of 3 bouts per day (1–2 minutes each) was associated with a 38% to 40% reduction in all-cause and cancer mortality and a 48% to 49% reduction in CVD mortality. A median daily duration of 4.4 minutes was associated with a 26% to 30% reduction in all-cause and cancer mortality risk and a 32% to 34% reduction in CVD mortality risk, mirroring results seen in exercisers engaging in structured vigorous activity.
Why it matters
These findings suggest that substantial mortality reductions may be achievable through very short, incidental bursts of vigorous activity embedded in daily life, providing a feasible target for individuals unwilling or unable to participate in structured exercise.
Limits
The observational cohort design cannot establish causality or eliminate residual confounding and reverse causation. Activity was captured over a single monitoring window rather than tracked longitudinally, and the UK Biobank cohort is subject to healthy volunteer selection bias.
