What was done

This Cochrane systematic review (updated through June 2016) evaluated mass media campaigns designed to prevent tobacco use uptake among young people under 25 years. Authors searched the Cochrane Tobacco Addiction Group Specialized Register, MEDLINE, and Embase for randomized controlled trials, non-randomized controlled trials, and interrupted time-series studies comparing mass media interventions (such as television, radio, print, billboards, or social media) alone or combined with school programs against education or no intervention. Outcomes included objective or self-reported smoking behaviour. Findings were synthesized narratively, and evidence certainty was evaluated using GRADE.

What was found

Eight studies were included (7 controlled trials and 1 interrupted time-series). Three studies (n = 17,385) reported that mass media interventions significantly reduced smoking behaviour (one vs. no intervention, two as adjuncts to school programs). The remaining five studies (n = 72,740) found no significant effect (three vs. no intervention, one as a school adjunct, and one interrupted time-series of a social media campaign). Effective campaigns generally lasted longer (at least 3 years), featured higher intensity (at least 4 weeks with 167 to 350 broadcast spots), and integrated school curriculum components (minimum 8 lessons per grade), though some ineffective campaigns shared similar features. The overall certainty of evidence was graded as very low.

Why it matters

Despite extensive investment in mass media public health messaging, the available evidence is too inconsistent and methodologically flawed to draw firm conclusions about whether mass media campaigns independently prevent youth smoking.

Limits

All included studies carried a high risk of bias. There was substantial clinical and methodological heterogeneity in campaign formats, intensity, populations, and study designs, precluding meta-analysis. Furthermore, only one study evaluated modern social media channels, leaving digital platforms under-investigated.
